    These situations are humorous, but the fact is in a situation like this there's nothing at all wrong with pulling the instructor (or your supervisor) aside and explaining that you know one another and it would be uncomfortable for you, and especially the patient, to go through this. The patient's feelings have to be taken into account. I know as a nurse myself I wouldn't want one of my colleagues to be involved in such an "intimate" exercise with me. You could always swap patients with another student or nurse to preserve the dignity of your client.
